Rehype plugin that wraps identifier tokens inside syntax-highlighted
<pre><code> blocks with <a> links to their API documentation.

Must run after a syntax highlighter (e.g. @shikijs/rehype) so
that code blocks contain <span> tokens to process.

Identifiers inside comments and string literals are never linked.
