Build a unified processor for rendering markdown to HTML.

The pipeline is:
remarkParse -> remarkGfm -> remarkBreaks -> remarkDirective -> remarkCodeProps -> [user remarkPlugins]
-> remarkRehype -> [rehypeTypedoc] -> [user rehypePlugins]
-> [rehypeTypedocCodeBlocks] -> rehypeStringify

If no rehype-typedoc options are provided, the typedoc plugins are skipped.
